Cellulite, clinically known as adiposis edematosa and commonly known as orange peel or cottage cheese thighs, is the dimpling effect some woman get on their thighs and buttocks. This puckering of the skin is caused by tension placed on fibrous bands amidst herniating subcutaneous fat. The connective fibers are attached just below the surface of the skin, and when fat deposits or fluid accumulates, it puts pressure on the skin and the bands. These bands do not fluctuate in length as the skin protrudes from fat or fluid retention, so the stretching of the skin causes small indentations where the bands are pulling down. These small dimples can appear on even the healthiest of women and are often nearly impossible to get rid of with diet and exercise alone.
Cellulite has many causes, including skin thickness, hormones, genetics, not enough physical activity, or slowed metabolism. Age is another potential cause, slowing the production of estrogen, blood flow, and therefore collagen. These factors can lead to the breaking of older connective tissue, which makes subcutaneous fat susceptible to pushing against the skin rather than being evenly constrained. And while subcutaneous fat is a main cause of cellulite, women of all sizes can be subject to dimpled skin on the thighs and buttocks, such as the aforementioned causes like skin thickness. It is also thought that tight elastic on underwear or stockings can contribute to decreased blood flow and in turn the formation of cellulite.
Cellulite does not appear the same for everyone. This is because the cause is not the same for everyone, and each women’s physique differs. Cellulite has been categorized into 3 levels.
- Level 1: No visible dimpling but examination at a microscopic level will show structural changes.
- Level 2: Visible dimpling when standing as well as structural change noticed at a microscopic level.
- Level 3: Pronounced dimpling and ripples when sitting or standing. Visible exterior roughness and structural change at microscopic level.
Men do not get cellulite dimples as women do because their fibrous tissue pattern is criss-crossed horizontally along the muscle, so it does not pull down on the skin. Women’s fibrous tissue pattern however is vertical, making them much more susceptible to getting the dimpling effect. It is also more common for women to get cellulite than men because women tend to store fat in the hips, thighs, and buttocks, while men store fat in the abdomen.

Who Qualifies as a Candidate?
Adult women without any serious health conditions who suffer from cellulite on the buttocks, inner or outer thighs can qualify as candidates for a Cellfina cellulite reduction treatment at our North Carolina location. The only contraindications for treatment are varicose veins and skin infections.
It is a misconception that thinner women do not get cellulite; this is not true. Women of any size can have cellulite because it doesn’t directly related to amount of fat or weight (although large fat deposits may increase the visibility of dimples.) Thin women can get cellulite as a result of genetics or thin skin.
Candidates for Cellfina will find that no amount of exercise or healthy diet can counter the dimples. It will worsen though if the patient does not lead an all around healthy lifestyle.
Preparing for Treatment
There is not a lot of preparation necessary before treatment with one of our North Carolina Cellfina experts, but there are a couple of things to avoid. Herbal supplements and anti inflammatory meds may cause bleeding, so it is best to not take them for about 2 weeks before the treatment date. Antibiotics may also be prescribed on an individual basis to be taken prior to treatment. Cellfina Treatment Process
A Cellfina treatment sessions takes a short hour to complete. Performed in-office, patients are able to return to their normal routine right after treatment.
Treatment begins with Dr. Saini carefully marking each of the dimples that will be treated. Once these dimples have been marked, an anesthetic is injected into the treatment areas to improve patient comfort during the procedure. Once the skin has been sufficiently numbed, a suction cup is attached to each of the dimples to stabilize the treatment area. Dr. Saini then inserts a tiny needle into the skin of each of these dimples and carefully cuts the fibrous bands of connective tissue. Gauze will be placed over the entry points and a compression garment will be wrapped around the thighs and/or buttocks.
Once the treatment is complete, patients experience virtually no downtime. Tylenol is often enough to manage any discomfort felt after treatment. While side effects like bruising, swelling, soreness, or tenderness are sometimes experienced, these usually subside within a few days of receiving treatment.
Cellfina vs Other Treatments
Liposuction is a fat removal procedure; it is not a cellulite reduction treatment. The removal of fat deposits can improve the appearance of skin dimples, but it will not remove them. This is because the connective fibers are still in place. Liposuction can be performed in conjunction with Cellfina for a slim and smooth contour if the patient desires.
Unfortunately, there are some topical skin creams and serums that claim to have the ability to remove cellulite. None have been proven or approved by the FDA. Some demonstrate the ability to temporarily reduce the appearance of cellulite such as Retinal. This is because it momentarily thickens the skin to camouflage the dimples, but frosting on a cake that’s lumpy can produce the same effect; it’s not a true solution, just a coverup. A true solution is resolving the underlying issue which is the tension on connective fibers, such as Cellfina does.
Typically, it takes about 2-3 months for patients to begin to see significant changes in the appearance of their cellulite. The FDA has recognized Cellfina as providing results for two years. This is not to put a limit on results, however, because some patients claim that they’ve enjoyed smooth thighs and buttocks for longer.
Studies have shown an 85% patient satisfaction rate at the three month mark, and jumping to over 93% come the following years after Cellfina treatment.
